Part of the inspiration for Red John may have been Keith Hunter Jesperson, a serial killer also known as the "Happy Face Killer". He would send letters to police and prosecutors and sign them with a smiley face.
It's currently unknown how Red John got his nickname, but given his narcissism, it's likely that he named himself somehow. The name was revealed to the public following the trial of RJK's accomplice, Orville Tanner. The possible inspirations for his name are: John Doe (name used for an "unknown and random man"), red blood or red hair, Red Dragon by Apocalypse of St. John (illustrated by William Blake), John the Baptizer, Gospel of John, first name of German composer Johann Sebastian Bach (his favourite musician).
